KIDS AT HEART | VOLUNTEER TUTOR ENROLLMENT FORM
Thank you for your interest in volunteering for the Kids At Heart program to make a positive impact on a student's life! The Kids At Heart Program provides tutoring and mentoring services for low-income students in grades Kindergarten through 12th grade. Clients can receive virtual tutoring services or in-person tutoring at partnering tutoring sites. Volunteers commit to tutoring one student for a minimum of one hour a week during the academic school year. 

Step 1. Schedule a Phone Interview
Schedule a meeting with the Program Manager, Carla Dorantes, at 619 - 376 - 3513 or carlad@upliftsandiego.org, who will walk you through our volunteer opportunities and determine what would be the best fit.

Step 2. Fill Out the Application Form
After meeting with the Program Manager, complete the tutor application form below.

Step 3. Complete the Volunteer Eligibility Steps
Complete a live scan criminal background check and online Mandated Reporter Training.

VOLUNTEER ELIGIBILITY: UPLIFT collects basic eligibility information required of Youth Service Organizations. All information is confidential and will only be used to determine suitability to volunteer as a tutor for the Kids At Heart Program.
